At Salud Healthcare, we work primarily with primary care providers through value-based care arrangements, especially within Medicare and Medicare Advantage programs.
In this model, health plans (like CMS and private insurers) estimate what it would typically cost to care for a patient. Providers are then empowered to deliver high-quality, coordinated care that keeps patients well and avoids unnecessary tests or hospital visits. When they’re able to do that effectively, physicians can share in the savings. The result is better outcomes for patients, and a smarter, more sustainable healthcare system for everyone.

What is CareConnect by Salud Healthcare?
CareConnect by Salud Healthcare is a dedicated call center support team that works hand-in-hand with your doctor’s office to make sure you get the care you need, right when you need it. You might hear from us to help schedule important appointments like your Annual Wellness Visit or a follow-up after a hospital stay.
When we reach out, it’s always on behalf of your provider, and you’ll hear from the same Care Coordinator each time—someone who knows you, understands your care plan, and is here to help.
